costs for emergency ambulance care.

Your ambulance service includes all medically necessary* ambulance transports, 24-
hours a day, 365 days a year, and covers the entire uninsured portion of your ambulance
bill. Most insurance companies only cover about 75% of your bill.

Guidelines for Medical Necessity are:

· Transport as a result of an emergency situation, e.g., accident, injury, or acute illness.

· Patient requires restraint

· Patient is unconscious or in shock

· Patient has to remain immobile due to fracture or possible fractures

· Patient has sustained an acute stroke or heart attack

· Patient is bed-confined before and after the ambulance trip

· Patient can only be moved by a stretcher.

* These services are covered 100% to members when physician authorized.

Membership Does Not Cover:

· Non-Medically Necessary Transports

· Transports from the hospital to your home

· Transports to or from your physicians office

· Transports to the hospital for non-urgent conditions

· Scheduled repetitive transports
